* In ''Manga/The100GirlfriendsWhoReallyReallyReallyReallyReallyLoveYou'', this is how the Kissing Zombies story arc begins. Kusuri gives everyone two flasks full of tea, then leaves to use the bathroom. Before she goes, she tells them that one of the flasks is for Rentarou and one is for the girls, but fails to mention that the ''big'' one is the one for Rentarou. Naturally, the girls assume that the big flask is for ''them'', and become victims of the LovePotion that Kusuri put in Rentarou's tea, causing them to become fixated on kissing him.

** In ''Anime/SuitePrettyCure'', Hibiki and Kanade's friendship is shattered prior to the series starting because they told each other that they'd wait for each other by a tree at the school entrance on the first day of school. Their school has ''four'' entrances and they didn't say ''which'' one. It takes a year, the two become Cures, and two children doing the same thing to realize where they messed up. Episode 6 is no excempt neither, Souta would never run away from home and then being involved on a Negatone attack, had not Kanade spend all that time scolding and being a jerk towards him for the mess he has done in the kitchen while making a present for her for White Day, instead of understanding his good intentions.

Unfortunately, Shizuku's rather harsh way of giving criticism only ended up hurting Ruri's feelings, but [[ExtremeDoormat Ruri]] never called her out on it, and only [[ApologizesALot meekly apologized]]. Partly as a result of that, Shizuku became even more abrasive toward Ruri, accusing her of holding her classmates back, and assuming she was skipping class. This culminated in Shizuku inadvertently causing Ruri to fall in front of the rest of the class, resulting in Ruri's best friend Seri slapping Shizuku and calling her a bully, resulting in the rest of the school ostracizing Shizuku. In reality, Ruri had a chronic medical condition that resulted in her being hospitalized, and while Ruri once weakly protested that she had to go to the hospital, neither she nor Seri mentioned that. Shizuku only finds out about Ruri's condition five years after the incident, when Seri mentions it in passing.
** After being ostracized, Shizuku became increasingly withdrawn, believing that she could only hurt others. Her parents and sister noticed the change, and got a dog partly to get Shizuku to open up more, but they didn't ask about her, nor did she confide in them- Shizuku mentally notes that she was afraid of what they would think. After going mountain climbing with her sister, Shizuku realizes how much her family cares about her and apologized to her mother for making her worry.

Ichigo's father and Urahara never tell him that they're [[spoiler:working on a way to restore his powers]], instead just having secret back alley meetings behind his back giving him every reason to feel paranoid and isolated. And one of the only major problems that DOESN'T come to pass, [[spoiler:that Soul Society has been secretly keeping tabs on him]], comes out well BECAUSE [[SecretTestOfCharacter Ukitake let gave Ichigo know about enough hints to figure it out ahead of time so Ichigo could reciprocate with a little trust.trust]].

* ''Manga/TheDangersInMyHeart'': During the work-study group trip to Akita Shoten, Ichikawa goes to a bathroom to vent his anger when he (wrongly) thinks Yamada and Kobayashi were making fun of him, with Yamada staying by to wait for him. The rest of the group didn't notice the two were separated until it was too late, leaving them behind at the train station. Both Yamada and Ichikawa pin the blame on themselves for not telling anyone. When they re-group, Kobayashi apologizes for not noticing.

* ''Manga/TheDangersInMyHeart'': Overall, a ''lot'' of the series’ conflict (comedic or otherwise) is derived from Ichikawa and Yamada's difficulty in communicating with each other and awkward, roundabout ways of doing so.

* In ''Manga/MariaNoDanzai'', Mari grows concerned that her son is hiding something from her, and shares her concerns with her husband Taiichirou on the phone. Since he's busy with work, he dismisses her concerns as overprotectiveness and assumes that Kiritaka is simply at an age where he wants more privacy. Turns out Kiritaka ''is'' hiding something: [[BullyBrutality he's being horrifically bullied at school]]. Because Mari's concerns aren't taken seriously, Kiritaka ends up jumping off a cliff after his bullies blackmail him into doing so, which [[CruelAndUnusualDeath culminates in his death after he's run over by a truck]] right in fromt of Mari, who worriedly followed him to the scene after he left the house in the middle of the night. Taiichirou later seriously regrets not paying more attention to his family, and blames himself for Kiritaka's death.

* Played for comedy repeatedly in ''Manga/SleepyPrincessInTheDemonCastle'': Princess Syalis is ''very'' bad at communicating, causing many a SustainedMisunderstanding when she either misunderstands what the demons are talking about or gives them the wrong idea about a situation by not explaining it clearly:
** When Hades asks her to deliver gifts to his brother Poseidon and report his reactions, she reports Poseidon's clearly positive reactions in ways that make them sound bad, such reporting his surprised "I didn't ask for this" as "He said he didn't want it", or stating that "he tore it up and threw it away" when she sees him excitely ripping up the wrapping paper to get at the gift.
** She's also prone to InnocentInnuendo, phrasing things in ways that sound lascivious when in reality they're anything but, such as asking various demons to "touch her in bed" when what she actually wants is a back massage.

*** ''Destiny'' is good at this. The entire conflict between the old and new cast could have been entirely avoided if both sides would at least have made an attempt to speak to each other. Kira and his friends intervene when Orb faces the Minerva to try to get Orb to back off, but after this fails Kira proceeds to just shoot at everyone (non lethally of course) without even trying to say anything to Minerva. Not even explaintion their intentions or a "Lay off of Orb and we'll help you get out alive." nothing. And Minerva instead of contacting Kira's side to ask what the hell they are doing, just kind of silently glares at them. Later on, when Zaft's invading Orb to get Djbril after the corrupt administration insists on protecting him, Kira and co show up halfway to take over control of Orb again, ordering everyone not currently fighting Zaft to go find Djbril to kick him out. But they don't tell Zaft this. Instead they keep fighting them, while trying to get Djbril with limited manpower instead of trying to work something out. As a result Djbril gets away.
*** There is an unfortunate reason for this: Kira didn't trust Zaft. He had a good reason for this, and his suspicions were (somewhat tragically) proven correct in the end, but he couldn't find a way to explain the situation.

It isn't until Miorine realizes Suletta isn't having lunch with the rest of them does she realize something's wrong.

** The [[Anime/GenesisClimberMospeada Third Robotech War]], between the Invid of the Regis occupying Earth and the Robotech Expeditionary Force trying to reclaim it, comes entirely from both sides ''refusing'' to talk things out in spite of the Invid's occupation being relatively peaceful (as long as you don't oppose them or try to get your hands on Protoculture without their permission, they leave you alove, and they even repaired the damage done to Earth by the Zentraedi bombardment). This is actually {{Justified}}: the Invid Regis thinks of Humans as barbarians who can't understand the marvels of Protoculture and the dangers of having it (and is perfectly right about the dangers), while the REF has just won a very destructive conflict with the Regent's Invid, who devastated the Robotech Masters' former empire and their homeworld of Tirol, and have no reason to think that the Regis' faction, having suffered the same things the Regent's has at the hands of the Masters, are any different, especially as the Invid have installed a number of jammers that block outgoing communications so the REF has no idea how things on Earth ''really'' are until some are destroyed in the lead-up to their final offensive. [[spoiler:That and the REF was being covertly manipulated by the Haydonites, the ancient enemies of the Invid, but nobody knew until, during the final battle, the Regis recognized a few pieces of Haydonite tech being used by the REF.]]

* Motoyasu in ''Literature/TheRisingOfTheShieldHero'' ends up causing a major catastrophe through poor communication, attempting to solve a village's famine by giving them a magically-enhanced seed he found in a dungeon. * Downplayed in ''Manga/MyDressUpDarling''. Marin recruits Gojo to make a costume for her, and mentions the next convention she's attending is in two weeks. Gojo proceeds to run himself ragged researching, designing, and constructing such a garment to be ready in time. When he presents it to her, Marin is horrified -- she didn't expect him to have it anywhere near finished, since two weeks was a ridiculous deadline for someone building their first human-scale costume, but thought this fact too obvious to mention. To Marin's credit she's extremely apologetic for the mix-up and beyond appreciative she has a costume ready for an event she thought she'd just spectate.
