** Poor Vinsmoke Sora had the misfortune of marrying [[TheSocialDarwinist Judge]], and was forced to watch as her children were experimented on by her husband, starting with her oldest child and daughter, Reiju. When Judge decided to psychologically alter her unborn quadruplet sons against her will, it was the last straw. So Sora decided to secretly take a dangerous drug that reversed gene-altering experiments to undo what Judge did, but she only managed to save Sanji and made herself terminally ill. And when the bullying/abuse started, Sora was already too sick to intervene, and did not live long enough to see what Sanji would become.
** Reiju lived in terror as she watched her father and younger brothers torment and abuse Sanji, unable to intervene directly because she feared that they would go after her too, for defending a weakling, so she would always help Sanji whenever they were alone. When she learned that Sanji was left in the dungeons to rot, Reiju secretly freed him and helped him run away, her last words being to find people who ''love'' him and never come back home again. She also knows why her other brothers were assholes to Sanji and everyone else (Judge's EmotionSuppression experiments), but she can't do anything about it and loathes their lack of empathy. In the end, she has so much self-loathing over her terrible actions as a Vinsmoke that she wants herself and her family to die.
** Sanji experiences this for most of the arc, having to put up with his abusive family and fearing for the lives of both Baratie and his Straw Hat crew, knowing that if he does something that might appear unfavorable, his family and friends' lives would be forfeit. It didn't help that Judge blackmailed him into obedience, giving Sanji's brothers leeway to torment him again just like old times. Then he learns two bombshells: That Pudding is an evil liar who's supposed to kill him on his wedding day ([[HeelFaceTurn she gets better later]]), and his beloved older sister, Reiju, essentially wants to commit suicide by attending the wedding with her family, where they would be rounded up and executed after ''he'' dies.
** Katakuri went through a moment of this in his childhood -- he used to not care for his hideous appearance since he always beat up anyone who bullied him, but he learned that the same bullies resorted to attacking and horribly scarring one of his sisters in an attempt to spite him. He blamed himself for letting it happen, and since that day, he crafted a fearsome image for himself so no one in his family would get hurt that badly again.
** Brulee suffered this as a child; she was used by a group of bullies to get revenge against Katakuri for beating them up, horribly scarring her face while insulting Katakuri's freakish mouth. When she comes back home bleeding on a stretcher, she's obviously in pain, but she tries to keep a brave face and assure her beloved brother that he's perfect just the way he is. As an adult, she implied that she hates her scar and envies pretty-looking people, because it reminds her of how Katakuri beats himself for not protecting her every time he sees it.
** When he was younger, Perospero was also devastated when he found his younger sister Brulee coming back home on a stretcher, with a ugly, bleeding gash across her face. He also had to watch Katakuri go from a gluttonous, happy-go-lucky person to a stoic, fearsome warrior who would cover his fanged mouth and literally kill to protect his family. ** Bege starts fearing for Chiffon's safety after she is captured by Oven to be subjected to a traitor's execution, and is given a SadisticChoice -- either he, his infant son, and his crew leaves unharmed (like Chiffon wanted) which will kill Chiffon, or he'll try defend her and get killed in the process. [[TheChessmaster Being who he is]], Bege [[ISurrenderSuckers pulls a bluff]] and [[TakeAThirdOption blasts Oven in the face from a safe distance instead]], freeing Chiffon.
** Charlotte Amande also had to go through watching a hunger-crazed Big Mom suddenly appear on Nuts Island and destroy it in her search for wedding cake, and being unable to do anything but desperately plead for her mother to stop. And when she sees Luffy with a captive Brulee, she also tries to rescue her sister from the Straw Hat's clutches, only to be blocked by Perospero's candy powers when he tried doing the same thing. The next time readers see Amande, she's standing with the survivors of Nuts Island, looking forlornly at the destruction Big Mom had caused.
** Chiffon's life is one big TraumaCongaLine -- first, her younger twin sister Lola ran away from home on the day she was supposed to marry Loki, the Elbaf prince, ruining Big Mom's plans to become the Pirate King. Big Mom was so pissed off that she wants Lola assassinated, and because Chiffon is identical to Lola, she ends up taking the full brunt of her mother's rage every time they cross paths, always fearing that she would die the next time it happens. When Bege's plan to assassinate Big Mom goes belly-up, everyone narrowly escapes, and in trying repay the Straw Hats by helping Sanji and Pudding bake the wedding cake Big Mom wants, her own brother, Oven, tries to execute her on the reasoning that she's a "traitor" to the family, which would potentially lead to either Bege having to raise Pez by himself should she die, or the Firetank Pirates being utterly decimated should they try to rescue her. Fortunately, Bege was smart enough to pull a bluff and blast Oven in the face from afar, saving Chiffon and allowing her to reunite with her husband and child.
** Pudding, like Katakuri, was also bullied for having a physical deformity, except she had no one to defend her. Not even Big Mom stepped in, only making a token effort to tell her to grow her fringe so she could hide her third eye. It's implied that the bullying left her with severe self-image issues, as Pudding believes that no one could ever love an ugly monster like her.
** When the Tamatebako explodes and topples Whole Cake Chateau, everyone's sent into a panic as they start falling to their deaths, including [[TheStoic Galette]] who ''[[OhCrap screams]]'' as she falls; and Brulee becomes completely distraught as she watches her entire family almost get wiped out by this unexpected disaster. While it was lessened by Streusen converting Whole Cake Chateau into sponge cake, the same couldn't said about the people on the ground, who would be buried by the massive amount of food pouring in.

That being said, the only reason the Vinsmoke children are so strong is because Judge wanted warriors to help lead Germa 66 to greatness, and so he genetically modified all of said children. Sanji and his brothers in particular were modified in the womb to be born strong but lacking empathy, but only Sanji was saved because his mother's desperate plan to save all of her sons didn't work as she intended. Thankfully, Sanji TookALevelInBadass by the start of the series and continued to grow from there.

For civilians, the price of living in Big Mom's territory is paying a "soul tax" ''with your own life force'' every six months. And according to Brulee, if you try to leave Totto Land, you'll die. Then there's the fact that Totto Land and its islands are ruled over by the Charlotte family, who all seem like gracious hosts on the outside, but are really just a BigScrewedUpFamily whose members, especially [[AbusiveParents Big Mom]], are mostly homicidally insane to the point where they make their allies, the [[MightMakesRight Vinsmoke]] [[VirtueIsWeakness family]], look like the ideal family in comparison.
