I don't have the specs on the model of portable defibrillator / EKG monitor used in the later seasons (a Datascope M/D2J), but I will note that the defibrillator has a docking port for the monitor. Somebody with more specialized knowledge should check whether this docking port allows the monitor to read through the paddles (like the automation in a modern AED). At any rate, I don't recall ever seeing an EKG trace on the monitor without either (a) a basic set of RA/LA/RL/LL electrodes, or (b) the monitor in its docking port, and the paddles on the patient.

Oh, and Re: Ironic Name, (1) Mike Stoker was played by . . . Mike Stoker, and (2) Stoker was an actual L.A. County Fire Dept. engineer, who happened to have a SAG card: the LACoFD mandated, at least when Universal was still using an actual LACoFD Crown Firecoach as Engine 51, that it be driven by one of their own.

Since the first entry doesn't cite a specific episode, just "From Fourth Season onward", I don't have a problem with you removing it if you've watched the episodes and haven't seen them take magical EKG readings.

The Ironic Name entry is a correct use of that trope for that character. The real life stuff isn't relevant (and by the way, that info about Mike is on the trivia page under Cast the Expert).
