Am I the only one who thinks Bigger Bad (the former name for Greater-Scope Villain) would be a nice name for this trope? It was often mistaken for this after all.

In some cases, the diff between this and Big Bad seem rather arbitrary... for example, could someone explain to me how Palpatine and Vader are Big Bad and Dragon, but Saren and Sovereign are MBTM and Big Bad? The two sets of characters are in THE EXACT SAME SITUATION.

I know nothing about the context so you'd have to explain it to me.

That said, The Man Behind the Man is the entity who from the background steers the behaviour of the villains. The Dragon is usually more independent.

Does a Man Behind the Man have to have orchestraited or otherwise manipulated his predecesor? Because if this is the case several of these examples don't fit.

That's The Man Behind the Curtain. The Wizard was never the hidden power behind anyone else, he just pretended to have abilities he actually didn't.
