Every living creature that directly shares your bloodline is dead. Every living creature that is directly related to any of those creatures is also dead. So, first level of relation—black dragons who are directly in that black dragon's family. Dragons who share the same blood as that black dragon—so barring the possibility of that particular black dragon having bred with another species, the first level of relation killed by the spell only includes black dragons. The second level includes the Draketooth family, and as specified, it's direct, bloodline relation. Tarquin's wife was killed because her child was a direct, bloodline relation to the Draketooth line and so was she, by blood. A child between Tarquin and that wife would have no blood relation to the Draketooth line, so it would be a degree separate from the way V describes the spell. And even if it did? Elan has no blood relation to the Draketooth line whatsoever, and even if a hypothetical child between Tarquin and that wife (which would not have been a direct, bloodline relation to the Draketooth line) was killed, it would not retroactively have made Elan a bloodline relation to the Draketooth line.
