Edited by ArivneOne difference is that the Mook Lieutenant tends to be higher up in the hierarchy of the evil organization, not middle management. "Lieutenant" doesn't refer to the military rank (which can be said to correspond to middle management) but to the original meaning of "second in command" or "right-hand man" (of the evil overlord, that is). As the trope description says, the Mook Lieutenant can be the commander of an entire army of mooks.
