Kyon, Koizumi and Mikuru are shown walking in the streets. All of them are wearing street clothes.

Koizumi: Right now, Suzumiya-san is more bewildered than anything else.

Yuki and Kyon are in Yuki's apartment, sitting in front of each other.
Yuki: My deletion has been scheduled in three hours.
Kyon: How set is this?
Yuki: It is absolute.

Mikuru: and showed it with pride...

Kyon and Tsuruya are in route to North High but none of them are walking. Kyon is one step ahead of her but turned back to see Tsuruya's downcast expression, her head sightly lowered and looking to her feet.
Tsuruya: I'm not allowed to give up.

Tsuruya: knowing we will all have each other...

The whole SOS Brigade plus Tsuruya and another girl are in the clubroom. Realization, which quickly falls into despair dawns in the face of the other girl.
?????: I don't remember. I'm sorry ... I ... I don't remember!

Kyon: working together so we could realize...

Haruhi, Tsuruya and Mikuru are sit inside a car. Tsuruya is watching Haruhi with a completely serious and neutral look on her face, Mikuru looks sightly surprised and Haruhi absolutely confident.
Haruhi: You think I'm going to sit by and see how it can be done by Kyon's family and not take that as a challenge to do it better?

[no voiceover] Kyon: Big Damn Hero

Straightforward Flashback-1

He took a moment to study her features, while she shivered in fright, clinging to him. Her brown hair was shoulder-length, and very curly just at the tips - fairly distinctive, he thought. She was probably as tall as Yuki, but with an even slighter frame, not quite gangly, but not yet grown into her height. Her dark eyes stared up at him pleadingly.

Calculating from the heights of their head, she seemed to be about the same height as Nagato. As for weight, she should be lighter than her. The hair seemed like it had been dried with a hairdryer after a perm, curling up like a smiley face at the ends. This can be taken as a unique feature. I don't know if it is because the uniform is not too fitting, the uniform seemed rather loose when taking a closer look.

Also, there are a lot of additional little descriptions of Kanae across the story, but the only way to find them all would be re-reading K:BDH from the start. I'm going to label that as "The Plan Z B".
