Regarding the ruler of a hellish realm...

Who do you prefer them as? The first demon that has always ruled it from the beginning? Or a demon that climbed the ranks and usurped the previous ruler? Which do you think is more interesting as a villain and character?

It depends on the kind of story you’re trying to tell.

Since I’m assuming this character is a villain, with those two options you’re either casting the villain as part of the establishment, or a revolutionary. Depending on who your heroes are, and the moral you’re trying to convey, one may be more appropriate than the other. If the heroes are revolutionaries it may make more sense for them to oppose a villainous establishment, for example.

I was thinking more of something revolving around The Multiverse and any concepts related to it.

The demon who climbs the ladder works better in explaining how he/she got there and how the realm is run IMO
