Full disclosure: There were originally a couple lines of start-of-episode credits text in the top area of the image, but I got rid of them by copying the surrounding green background over them. Is that okay or should I have left them alone?

It's fine, we've modified images similarly before.
I mean, there might be a line to be crossed with regards to single-panel images and their copyright blurbs, but blanking the "this is an episode written by [scriptwriter]" text doesn't seem like it'd cross it.

3 is a good one, I agree, the eye shape is a wee bit more distinct. I am a bit overwhelmed by the fact that shows like MLP:Fi M already have a ton of images (this is coming from a brony), but whatever works works.
I wish I could find a picture of that one episode of The Venture Bros where Dr. Venture gets punched so hard that his right eye is dislodged from its socket. It showed an Eye Cam that was split between his left eye looking forward at what's around him, and right eye looking at the floor.
I'm not familiar with the show, but I noticed it was available on Netflix, so I used the recap pages to hunt down the scene in question. Turns out it's not even an actual example; it's a unique shot that has the screen divided down the middle, but the screen itself remains perfectly square. Wah- wah- wah- wahhhhhh.

Well, does the Eye Cam trope require seeing the blackness of the eyelids in the corners of the screen? If it it is that, and not just showing the world through a character, then alright then. If it is just any shot shown through the eyes of a character, then that example would count.
It does; the description specifically spells it out. If it's just a normally-shaped first-person shot (even if it is divided down the middle), it's P.O.V. Cam.

Check out my fanfiction!Personally, I think 3 and 10 illustrate the camera shape equally well, but as far as giving context is concerned, I slightly prefer 3 because having multiple visibly concerned characters staring at the subject makes it a bit more obvious what's going on.

A collage would be fine, but if it's done, I'd prefer it to have multiple media represented.
That's what I was going to say. A collage works best if it represents numerous media. Find an example of Anime, Film, Video Games, Comic Books, etc, and throw them together.
Well, it only took us about 2 weeks to get some candidates for western animation. At this rate, we should have our complete collage of different media by...sometime next year? (It's a recently launched trope, too, so the examples list is very limited; there are no anime/manga examples at the moment.)
Is this used in anime? I have never seen it.
Proteus would be good for video game as it is an actual mechanic to the game although it is not installed on my vita at the moment to take a screen.
The curvature isn't very pronounced in that one.
Its a pixel based game akin to Terraria everything in it is completely blocky, Sadly.
