What Do You Mean, It's Not Cosmetic? is an somewhat old and well-crosswicked page, but it has a line of dialogue title and only 5 inbounds - a common consequence of line of dialogue titles, especially when they are also snowclones. Which is a bit of a disappointing performance. Was discussed here a bit and was suggested to take it to TRS.
It looks like the description needs a little fleshing out as well.
Bump. Any action here?

None so far, and I don't like asking for a rename crowner with so little input.
OK, heads up: I play video games only very, very little, so I'm not exposed to tropes related to video games much. But this one could definitely use a new name and the description should be expanded — it mentions it looks like Cosmetic Award at first, so the comparison of the two could be elaborated on. Does it subvert the Cosmetic Award perhaps? Also, it needs a bit of clean-up with Example Indentation and general example misuse like this one: "Many MMORP Gs nowadays attach small stat bonuses to their Titles. Developers occasionally get carried away." It's my understanding that broad generalizations about recent development are not allowed to be posted.

Votes bump; renaming is at 5-1.
Another votes bump; now 11-2 in favour of renaming.
Will do the alt names crowner once I am back home.
Made the names crowner, although it can use better name suggestions.
Downvoted Not Just Cosmetic because it's broader than the trope.
That seems to lack the Cosmetic Award aspect, methinks.
I don't see how personally. Video games with an Achievement System basically offer nothing but cosmetic awards 99% of the time. It's the 1% that don't that comprise What Do You Mean, It's Not Cosmetic?. Hence the name I suggested.

Bumping for votes; right now, everything is in red. I dunno why.
I'm not really sure we need a separate trope for what is essentially a simple subversion of Cosmetic Award. But anyway, since the trope references Cosmetic Award, that is what the name should reference too. Not Achievement System. Also, not all bonuses given by achievement lists are necessarily examples. Achievements only "look like a Cosmetic Award" in games where most of them actually are.
This needs some more votes because right now nothing is winning.
Reality is that, which when you stop believing in it, doesn't go away.

Not Actually Cosmetic Award is leading.
Bumping to point out once again that Not Actually Cosmetic Award is leading.
Did the rename.
