There has been an accident a while ago in Switzerland (in the town of Wolfenschiessen) where a train overran a car that was crossing the track on a crossing. 3 fatalities. Apparently, that crossing didn't have lights and only the St Andrew cross marking.
Also a train of the Rätische Bahn at 12.45 when it was running across the Albula gorge was hit by a mudslide. Two coaches were thrown down into the gorge below and only stopped from crashing into the bottom by trees. Several injured people, no fatalities.

From what I could see it was a pair of EWS Class 67s top and tailing a rake of about four yellow coaches.
