While I agree this is JAFAAC, I can foresee difficulties in finding a suitable replacement because there's really no commonly known feature of a "mistress" for a picture to show.

Actually, I now see that Your Cheating Heart has an image pretty much exactly like what I meant.
Honestly, I'm very fuzzy on the difference between the two. The Mistress never really... defines itself. The example in the description of Your Cheating Heart seems to specifically covers The Mistress, in fact.

The Mistress is a character trope — a person. Your Cheating Heart is a story trope — a plot or situation.
While most situations involving The Mistress are also examples of Your Cheating Heart (I say "most" because it may not be if the marriage is open or the couple is poly), not all examples of Your Cheating Heart involve The Mistress (for example, one-night stands or short flings, or those where the "cheating" is less than a full-on sexual relationship, as is common in works for teens, where kissing or even simply flirting with someone else is treated as "cheating").
It's going to be very difficult to find a good image that conveys 1) He's married to someone else and 2) this is an ongoing relationship, not just a fling or a one-time thing.

I don't think there's any way to show that it's an ongoing relationship but it would be better to show a character that's young and sexy. He should be in the picture too.

Honestly, unless someone can find a lampshade, I'm good to call this a BUPKIS case.
If we do decide to keep the current, I suggest putting a comment on the page mentioning this and previous IP threads. Ask that nobody bring it to IP again without having a replacement to suggest. I don't know how effective it will be but it may help.
