I'd say both of them are poor images. Neither one looks particularly villainous, unless you know the work. That's Classic Just A Face And A Caption. The new one is, I'd say, worse, in that regard, since the only thing that indicates he's a villain is the caption saying he is and there's nothing to indicate he meets the "sissy" part of the trope at all.
The old one at least had some artistic conventions that suggest maybe he's not a Good Guy — the Shoulders of Doom / High Collar of Doom and the jagged edge treatment of the collar and the shaded upper half of his face are all conventions that usually indicate "villain". But the caption is doing all the work on the "sissy" part.
My vote is to pull both of them and find a good one.

He does look somewhat villanous, but the sissiness is rather lacking. It doesn't seem like it would be that hard to find a better one, at any rate.

Like Septimus said, I can see a little of sissiness in the original picture without needing the caption. It's not fantastic, but I think it does the job. Keep Until Better Image Suggested, which I don't think should be too hard to find.

I like both HIM pictures. I perhaps slightly prefer the first one because of the Slasher Smile. But either would be a great replacement.
