Didn't read it, but I saw something on Fanfiction.net that crossed over Percy Jackson with The World Ends With You. From what I could gather from the summary, Percy was playing the Reaper's Game with Luke as his partner.
I can't imagine why the author thought this was a good idea.
On the other hand, I saw a Phineas And Ferb and Sonic The Hedgehog fanfic that actually wasn't half bad.

One time, I found a Twilight / The Hitchhiker's Guide To The Glalaxy crossover. I swore I would read it (it actually appeared to be well-written in terms of grammar, punctuation, etc.), but when I checked again after I got home it was gone.

Personally, Winnie The Pooh and Star Wars. In fact, mostly anything with Winnie The Pooh if you're savvy enough with Youtube.
Although there was this one weird crossover I saw where it had Annabelle from Annabelles Wish turn into a deer to be Bambi's new mother... and some Thomas The Tank Engine-like Tugboats were involved for some reason.

Myself, two I just randomly ran into on Google searches. I didn't even click the links, I just saw the Google-snippets. One was between Super Mario Bros and Angel Blade (I wouldn't click that latter link at work, if I were you), and another between Rocket Power and Dragon Ball. There is of course... that other one involving Dragon Ball, but I'm pretty sure that's a Troll.
