It kinda is: it's from I Wanna Be The Guy. It tells you to take the sword, and when you do you die and get berated for the stupidity of walking into a sword. Not the clearest example, though.

I agree, we should pull the image, but not without having decided on a replacement image.
If I find a better image, I'll edit it in this post. Maybe just a picture of The Kid exploding with the caption, 'See how there's nothing harmful on the screen and you're dead? You just hit Q.' seeing as how hitting 'Q' in IWBTG makes you not-breathing.

I haven't played the game (and never will), but I'm not even sure this is an example. In any case, the caption is dangerously close to, "I Wanna Be The Guy shows us how it's done."

That's pretty good. It's actually an example, too.
It seems there is pretty unanimous consent to pull the old one while the new one is much more illustrative. (Would throw in my vote for the new one as well)
I changed the image (sorry if it's not the proper conduct to change it over so quick...I lurk more than I actually do things on T Vtropes) and removed the caption. Edit, reprimand, and correct me at will.
