  • Alternate Character Interpretation — So, so many.
    • Why does Shinji do the things he does the way he does them? Why does he conspire with Zenitsu on one case, then fight himin another. Is he for or against Gakuso, or simply against the Lucy Monostone experiments?
    • Machi, post-timeskip, is OOC because Wakana is seeping through now that there is nobody around to keep up appearances for or because she's shocked by Amamiya's disappearance?
    • Wakana, in the flashback, is shown to be posing as Machi even without Fake Memories, how much of what we've seen her do and say been in-character?
    • Does Tetora have an endgame or is he doing all that killing for shits and giggles?
  • Audience-Alienating Premise — While a detective series about a detective with MPD could work (look at Layton Brothers), one filled to the brim with Squick and Nightmare Fuel, some of which is perpetrated by the protagonist? Yeah, no.
  • Darkness-Induced Audience Apathy — After the Time Skip, you'd be hard-pressed to find a character to root for, or empathize with. Sasayama's chapters try to alleviate this, but his Butt-Monkey status makes it an uphill battle.
  • Nightmare Fuel — The entire series, pretty much cover to cover. You know those small "Explicit Content - Parental Advisory" stickers that cover most mature manga? For MPD Psycho, it fills half the front cover!
    • Although, the certain volumes that do have this sticker also have mutilated, naked women on them.
  • What Do You Mean, It's for Kids? — Can you believe this series ran in a monthly Shōnen magazine for years, for exacttly 11 Volumes worthy of content before moving to a temporary then a permanent Seinen magazine?
