- Alas, Poor Scrappy: This was one of the first episodes in which Brian was made to look overwhelmingly sympathetic in an effort to win back his affection.
- Heartwarming Moments: This episode is full of them in both the A-story (Stewie worried over Brian dying from the kidney operation) and the B-story (Meg telling Chris what hope is, which is basically a summary of her life — how she still holds out hope for a better life, despite not having it now).
- Hilarious in Hindsight: When Peter misses dialysis, his skin turns yellow and he mentions how he can keep going another 20 years (a Shout-Out to The Simpsons). As of 2019, Family Guy itself has been on for 20 years and counting.
- Nausea Fuel: Both in-universe and out. Stewie kidnaps Brian and takes him to the park, where he tearfully demands that Brian not go through with surgery, lest Stewie lose his best friend. It's all really quite sad... until Stewie's nose starts running profusely and he ends up covering Brian in snot when he hugs him against his will.
- Unintentionally Unsympathetic: While the episode tries to play the dilemma dead seriously for a change, ultimately Peter's health issues are still down to his own stupidity and repeatedly ignoring advice (including Brian himself), meaning Brian almost had to sacrifice himself just to prevent Peter's Too Dumb to Live behaviour costing him his own life.
