- Complete Monster: Collectively, the global conspiracy. Killing and mutilating babies and disposing them in a town dump. Check. Employing people to rape children into insanity. Check. Concentration camps where they torture people. Check.
- Confirmation Bias: Those who share O'Brien's views tend to be one of his bigger fans.
- Fridge Horror: Regardless of how one views O'Brien's writing, the sort of world his characters (and apparently the author himself) envisions for wouldn't sit too well with many.
- High Octane Nightmare Fuel: The town dump, where the global conspiracy disposes of the mutilated bodies of babies. Quote: "Very small arms and legs. Heads and faces too."
- Additionally, Pawel's molestation by his great-uncle.
- Moral Event Horizon: Ashley's refusal to believe in his son Nathaniel's innocence when the latter is framed by the government conspiracy. Made even crueler by a flashback, in which, despite their political differences, Ashley promises to stand up for Nathaniel should his views get him in trouble with the government. Instead, simply because of political disagreements, Ashley prefers to believe his own son is a pedophile and a murderer.
- Self Plagiarism: Several of Nathaniel's articles in Plague Journal, have appeared in Real Life publications, under O'Brien's byline.
- Squick: The dump scene.
- Also, when the medicine man has sex with Maya, he suggests that she wake her eight-year-old son, Arrow, so he can watch.
- Additionally, Pawel's molestation by his great-uncle.
- The Woobie: Rose, Arrow, and Pawel.
