In a nutshell, Spider-carnage and the destruction of the multiverse is how things turned out in animated series timeline before the intervention of Madame Web and the Beyonder. This would explain the kind of out of place stinger with Doctor Warren finding a fragment of Peter's costume to use in cloning, and why Madame Web was so set on Peter being "the Chosen One" who could resolve this crisis; he wouldn't just be fighting an alternate universe version of himself, he'd be literally fighting his exact self from a different future of his own timeline.

An alternate version of the theory above; both Spider-Carnage and Ben Reilly were clones created by Miles Warren immediately after the events of the Hydroman two parter. The real spider-man was taken away by the Beyonder to participate in the Secret Wars, with at least a year and several months passing. In that time, Ben and the Peter who would become Spider-carnage were both created and released into the world; they both took the absent spider-man's place fighting crime, and ultimately things devolved into the situation we see when the real Spider-man returns. Of course that means that Spider-Carnage only comes into being and threatens the world because of the Beyonder and Madame Web's intervention.

This is a relatively small and inconsequential theory, but it's possible that Silvermane hired the guy who shot JJJ's wife. The two actually did have a history in the comics (in a Spiderman annual, Silvermane actually did come to the Daily Bugle and threaten JJJ and its employees about a story made about him) and Jameson did say that the crook shot his wife due to him refusing to back down on a story about a crime boss. Plus Silvermane is the right age to have been active as a criminal during that time, unlike the other crime lords in the series.

If the Venom symbiote did not originate on the Moon in this adaptation, chances are that it may have been banished as an outcast, like in the comics. Venom and Carnage could have easily been transported to the symbiote homeworld itself, and if Spider-Carnage's symbiote is not an alternate dimension's version of the Carnage symbiote, that means it was banished much like Venom (or perhaps ''by'' Venom)...and thus left Kasady to die.
