First Name Basis"... that trope is about a change to a more famililar address, so if they used to call each other something else, could you specify what, and the circumstances (otherwise it doesn't fit here)? Not sure about the rest. Dividing the examples by chapter is a good idea, but the chapter descriptions might do better at the top...
