Seth: Isnt the second example Dead Star Walking not a subversion of Contractual Immortality?
Rincewind: Never edited the wiki before, so I figure I'll just drop it in here. The last bullet point for the Rincewind example suggests the Spell in his head as his survival reason, but that leaves by, what, book two? The Light Fantastic, I believe, when he says all the Spells and hence gets rid of it. So that only applies for the first two books.
Scifantasy: Removed the Girl Genius example, because it's really not accurate...
