- Dead All Along: It's a bit of a shocker when the players go through Lamm's treasures and find her head.
- Heroic Sacrifice: In the final adventure, she sacrifices what's left of her on the material plane in order to aid the PCs.
- Mama Bear: Went after Lamm for killing her son. It didn't end well. She's using the players to try again.
- Posthumous Character: She's already long dead when the Adventure Path begins.

- Captain Ersatz: He's pretty much the Sewer King from Batman: The Animated Series right down to the pet alligator.
- Child Soldiers: He has a bunch of enslaved orphans he forces to fight for him (though presumably good characters will use nonlethal measures)
- Evil Old Folks: A real scumbag of a man who's lived at the borders of society for decades.
- The Fagin: A particularly dark, cruel, and violent example. He is horribly abusive to the captive orphans he uses as pickpockets and slaves, and he almost invariably kills them when they're of no further use.
- Hate Sink: He's a thief,slaver,child abuser,animal abuser etc. It's unlikely the players will view him too favorably.
- Hoist by His Own Petard: He starts combat by shooting his own pet alligator to make it mad. If he's forced to retreat through the gator's den while it's still alive, things likely end badly for him.
- Starter Villain: He has no connection to the plot aside from being the one who brings the player characters together.
- Warm-Up Boss: He's pretty easy even for level one characters, who should be able to quickly give him a well deserved death.

- Big Good: She's the most constant and reliable NPC the party comes to rely on and her overall devotion to the city and its people above the crown make her very beloved by the people. So much so that the canon epilogue to the story has her becoming the new ruler of Korvosa after Illeosa's death.
- To Be Lawful or Good: She starts off as Lawful as she wishes to keep order in the city above all else, even if it means having to deal with a Tyrant like Illeosa in charge. When pushed to the brink however, she decides to switch to the latter and starts a full-blown rebellion when the depths of selfish destruction of the queen are revealed.
- Reasonable Authority Figure: She's one of the few sane people in the current Korvosa government.
Marcus

Alignment: Lawful Good
- Honor Before Reason: Cressida notes that he tends to act this way.
- Leeroy Jenkins: Ultimately gets him killed.
- Spared by the Adaptation: The Updated Re-release has Ileosa only badly wound, rather than kill him, and he can be rescued when the heroes raid the Deathhead Vault.
- Walking Spoiler: His involvement with the plot begins and ends with his death.
- The Worf Effect: He's a CR 13 combatant, but he's effortlessly killed by Ileosa mainly to show off her power.
Queen Ileosa

Alignment: Neutral Evil
- And There Was Much Rejoicing: After her death, a celebration of her downfall was established in Korvosa. Called the Crimson Festival, it involves burning wooden effigies depicting her, and her statue getting pelted with rotten fruit and soiled by house drakes.
- Aristocrats Are Evil: Even before becoming queen she was a petty bitch.
- Artifact of Doom: Dons the Crown of Fangs, which contain a piece of Kazavon's soul.
- Assassin Outclassin': The captain of the Sable Company tries to assassinate her by shooting her in the head with a crossbow. She doesn't even flinch, and stabs him between the eyes with his own crossbow bolt.
- Big Bad: Of the Curse of the Crimson Path Adventure Path.
- Bitch in Sheep's Clothing: Presents herself as an innocent woman in mourning for her husband, but is in reality anything but.
- The Caligula: Ileosa seems to think that Korvosa is her toy, and that she has the right to do whatever she wants with it. Her rule gets more and more tyrannical and inhumane as time goes on.
- Came Back Wrong: In Shadows Over Sundown Lorthact’s last ditch gambit creates several imperfect resurrections of Ileosa who the players must fight. Defeating him however makes this Came Back Strong.
- Demonic Possession: She was influenced by the spirit of an evil dragon, Kazavon. But not completely, because it's More than Mind Control.
- Depraved Bisexual: As well as her marriage to the late king, there are rumors of a relationship with her female bodyguard. Word of Gay confirms that she is bisexual.
- The Ditz: Before Kazavon's influence increased her competence, she had an Intelligence and Wisdom score of 9 and 7 respectively.
- Evil Is Petty: Pre-possession. Even post-possession, her goals are still exceptionally petty compared to the body count she's willing to cause.
- Evil Sorcerer: She's an evil Bard, whose powers are amplified exponentially by Kazavon's fangs.
- Final Solution: Decides to use Blood Veil to kill off the Varisian and Shoanti populations of Korvosa, because her vanity causes her to hate the idea that not everyone in the city looks like her.
- From Nobody to Nightmare: Sure she was Queen of Korvosa beforehand, but she was such a spineless coward that it wouldn't have made any difference. Then she found the Fangs of Kazavon and became a fully-fledged monster.
- God Save Us from the Queen!: Queen of Korvosa and Big Bad of the Adventure Path.
- Gold Digger: She married King Eodred solely to gain the wealth and prestige of being a queen. She was widely disliked in Korvosa because of this even before she became the city's ruler.
- Healing Factor: To the point that a headshot with a crossbow doesn't even phase her.
- Immortality Immorality: Plans to murder thousands of people to achieve eternal youth.
- Kill the Poor: Unleashes a plague with this goal in mind.
- Manipulative Bastard: To the point that the book notes that it's entirely possible that even if she's killed she'd manipulate an archdevil into letting her out of hell.
- Navel-Deep Neckline: Her dress has a plunging neckline down to her belly.
- A Nazi by Any Other Name: Her plans to kill all the "unnecessary" people in her kingdom seem rather like the T4 program in Nazi Germany. Bonus points for most of her intended victims being Varisian (the Golarian version of Gypsies/Romani)
- More than Mind Control: While the Fangs of Kazavon did implant a shard of the evil dragon's spirit in her, it didn't take control over her. Rather it just gave her a huge confidence boost.
- Pimped-Out Dress: Wears one to Trinia's execution (and is seen wearing it several other times during the campaign).
- Redhead In Green: Her original outfit is mostly green, though she switches into a magical red gown about halfway through the campaign.
- Rich Bitch: Originally, though she quickly goes from this to a full-on monster.
- Sorcerous Overlord: By the end of the campaign, she is a very powerful bard whose great magical power is further enhanced by her bond with Kazavon and her bargains with devils, all of which she uses to tighten her grip on Korvosa.
- Took a Level in Badass: Goes from sixth to twentieth level over the course of the campaign, largely as a result of Kazavon's power.
- Vain Sorceress: Desires to become immortal and stay beautiful forever.
- Vampire Monarch: Returns as a vampire queen in the aftermath of Shadows Over Sundown.
- With Great Power Comes Great Insanity: The Crown of Fangs gave Ileosa great power at the expense of her sanity.
- 0% Approval Rating: Without a doubt the most hated ruler in the history of Korvosa. Even her closest allies end up resenting her too much to stay on her side.

- Arc Villain: For Seven Days to the Grave, along with the Cult of Urgathoa.
- Comic-Book Fantasy Casting: He looks almost identical to Nicolas Cage.
- Evilutionary Biologist: Helped create the blood veil.
- Morally Ambiguous Doctorate: It's clear that this guy doesn't care for the whole "do no harm" thing.
- Professional Killer: He's a member of the Red Mantis, though not a full assassin.

- Arc Villain: For Seven Days to the Grave, along with Dr. Davaulus.
- Eerie Pale-Skinned Brunette: A beautiful but disquieting looking woman with alabaster skin and midnight black hair.
- One-Winged Angel: After she's defeated, Urgathoa transforms her into an undead monstrosity which the heroes have to put down again.
- Plague Master: She's a mad cultist of the goddess of disease, and had created the deadly plague known as blood veil.
- Sinister Scythe: Her main weapon.
- Smug Snake: Treats the heroes like a bunch of idiots and gloats about her invincibility during their confrontation with her.

- Fat Bastard: Usually possesses a bloatmage named Togomor, who is so corpulent that even his Imp familiar is disgusted by him.
- Deal with the Devil: Helps Ileosa make one.
- The Dragon: To Ileosa, to the point that he basically runs Korvosa while she's at the Sunken Queen.
- Dragon with an Agenda: Working to serve Lorthact's goals, not Ileosa's.
Jolistina

Class: Rogue/Sorcerer
Alignment: Chaotic Evil
- Captain Ersatz: Possibly, she does have a lot in common with Harley Quinn.
- Mad Love: With her necromancer boss.
- I Love the Dead: Implied, she has 21 zombies under her command (who originally died from some disease the queen spread on purpose.) The book says she likes to "play" with them, but is running out of remove disease spells. What such "play" involves isn't explained, but it seems pretty obvious what they're suggesting.
- Perky Female Minion: To Rolth Lamm.
- Psychopathic Womanchild: For being a century-old elf, Jolistina is very immature and childish, and gleefully serves a mad as deranged as Rolth Lamm.
- Too Kinky to Torture: It's noted that she's a masochist, and attempts to use Intimidate on her only excite her. Unless they involve denouncing her failure to her boss.

Race: Blue Dragon
Alignment: Lawful Evil
- Animalistic Abomination: While it's rather ambiguous if he actually was one, the thing to keep in mind is that his power came directly from Zon-Kuthon, and according to the opening notes of the Relics of Kazavon chapter of CotCT book 6, he was regarded as such by other mortals.
- Artifact of Doom: After Kazavon's death, seven powerful evil artifacts that still contained his wicked spirit were made from his remains. They can corrupt those they touch, and if all seven are assembled, can be used to resurrect him.
- Axe-Crazy: Exceptionally so. Kazavon lived for nothing but the torture and slaughter of the innocent, setting up a kingdom that was little more than a pool of victims to sate his bloodlust.
- Back from the Dead: If you screw up at the end of the Adventure Path. Have fun with the CR 25 Blue Dragon.
- Breath Weapon: Like all blue dragons, he can spit lightning.
- The Corrupter: Each Artifact of Doom has this ability, inflicting the user with a case of With Great Power Comes Great Insanity.
- The Champion: Of Zon-Kuthon.
- Dark Messiah: Kazavon was empowered by the God of Darkness to be his greatest weapon.
- Demoted to Dragon: His disembodied spirit, using the Everdawn Pool to create a weaker body for himself, fights by Ileosa's side as a guardian.
- The Dragon: To Zon-Kuthon, in the most literal sense of the term.
- A Fate Worse Than Death: The Church of Zon-Kuthon believes him being bound to someone as petty as Ileosa is this for him, though his own opinion on the matter is never stated.
- After his death the Neutral Good wizard Kolwyddon tried to destroy one of his relics, specifically the Staff Of The Slain, for good. Unfortunately, he was not only unable to do so, but got trapped in the Staff along with Kazavon's own spirit, where he remains to this day.
- Greater-Scope Villain: One whose threat doesn't end with Ileosa's defeat.
- Haunted Castle: His stronghold, Scarwall.
- I Love the Dead: While never stated outright, the gruesome artwork his spirit inspires and mentions of "vampiric orgies" at Scarwall strongly imply that Kazavon was a necrophiliac.
- Manly Facial Hair: He had a massive mustache in human form, as part of his Vlad the Impaler homage design.
- The Man Behind the Man: To Ileosa, though the degree to which he's influencing her is never explicitly stated. Word of God indicates that all he really did was give her the intelligence and confidence to do what she wanted to do anyway.
- The Mole: In the Backstory, Kazavon disguised himself as a human warrior and helped a duke reclaim his lands from rampaging orcs. When the land was secured, Kazavon took the land for himself and began building his kingdom of darkness.
- More than Mind Control: As noted above, Ileosa willingly falls under his sway when she comes in contact with his preserved fangs.
- No Historical Figures Were Harmed: He's essentially Golarion's Vlad the Impaler (technically, he is literally the son of a Dragon). His human form even looks like him.
- Posthumous Character: He's dead... technically.
- Psycho for Hire: He posed as a mercenary, and essentially works for Zon-Kuthon because of the opportunities it gives him to hurt people.
- Sadist: Against unsatisfactory minions as well as his enemies.
- Sealed Evil in a Six Pack: After his defeat, Kazavon’s remains were hastily forged into seven relics, each containing a fraction of his being, and taken VERY FAR AWAY from each other.
- Shock and Awe: Like all blue dragons, Kazavon breathes electricity.
- Soul Jar: Seven of them. They basically act as Artifacts of Doom.
- Spikes of Villainy: His spines are much more pronounced than in other blue dragons.
- Voluntary Shapeshifting: He could take on a human form, and actually preferred his human form over his dragon form; he found that people were much more likely to willingly follow a cruel tyrant than an evil dragon, and he found the sensation of blood on soft skin to be more appealing than feeling it on hard scales.

Alignment: Lawful Evil
- Affably Evil: Her chronically cheerful, happy-go-lucky attitude clashes mightily with her being a priestess of the god of pain and torture, going so far as being ostracized by most of her church. She is certifiably insane but very friendly towards the party and is a possible powerful ally.
- Black Eyes of Evil: Though that's because of elven genetics rather than evil.
- Epic Flail: She fights with a spiked chain.
- Genki Girl: A stark contrast with most worshippers of Zon-Kuthon.
- Heel–Face Turn: If she survives Scarwall, she leaves the church of Zon-Kuthon.
- Impossibly Cool Clothes: In the pictures, her armor seems to be a hooked chainmail body stocking. This is changed for the Updated Re-release, since Word of God said that no one liked her original look and it being included in the final product was a miscommunication.
- Sinister Minister: Priests of Zon-Kuthon are not a stable lot.

- Grave Robbing: How he's first introduced.
- Kavorka Man: One has to wonder just how Jolistina fell for him considering his looks and personality.
- Obviously Evil: His face is covered in sores, scares, and pustules.
- Psycho for Hire: He goes along with the Cult of Urgothoa's plans For the Evulz.
- Serial Killer: For a period he was the infamous Key-Lock Killer, one of Korvosa's most feared murderers.

Alignment:Lawful Evil
- Abusive Parents: At age eight, her mother put a spell on her that forced her to kill someone every month or endure horrible pain.
- Arc Villain: For A History of Ashes.
- Broken Bird: Her abusive childhood has left her an emotionless killer.
- Demoted to Extra: In the anniversary edition, her role as the final boss of book 4 is reduced to a mini-boss fight beneath the Longacre building.
- Evil Redhead: Though it's closer to pink Depending on the Artist.
- Professional Killer: As is standard for the Red Mantis, she combines this with Church Militant.
- Spared by the Adaptation: The anniversary edition gives her some loose redemption mechanics, as opposed to her fighting to the death in the original version.
- The Stoic: She's become numb to emotion due to her traumatic childhood.

- Boisterous Bruiser: Once his respect is earned.
- Defrosting Ice Queen: He starts off hating the player characters, but if they play their cards right he comes to respect them.
- One-Handed Zweihänder: He fights using the Shoanti "Thunder and Fang" style, and so wields his Earthbreaker with one hand.
- Secret Test of Character: Most of his antagonism of the player characters is in order to assess if they're really as bad as other outsiders he's met.

- Bait-and-Switch Boss: The players seem to be building up to an epic confrontation with her, mounted on a black dragon, until she switches sides and helps them kill the dragon.
- Bodyguard Crush: She's in love with Ileosa, and has a secret relationship with her, though she comes to realize that Ileosa doesn't love her back.
- Good Scars, Evil Scars: Like every member of the Grey Maidens, her face was scarred as part of her initiation, however the scars are confined to one side of her face and, in the original illustration for the Adventure Path, don't look too severe. The latter is changed in the anniversary edition, and her face looks much more badly damaged.
- Heel–Face Turn: In the end, Ileosa's atrocities become too much for her to bear, and she switches sides.
- Incompatible Orientation: Both Vencarlo and one of his students were interested in her, not realizing that she was more interested in women.
- My Master, Right or Wrong: Up until her breaking point.

Alignment: Chaotic Good
- Badass in Distress: During Escape From Old Korvosa.
- Passing the Torch: He is looking for a worthy successor to pass the Blackjack legacy to. If a PC can meet his standards, he'll encourage them to take on the identity.
- Stealth Mentor: He's sizing up the PCs to see if any of them has what it takes to take up the mantle of Blackjack.

Class: Bard
Alignment: Chaotic Good
- Face Death with Dignity: The Queen blames her for killing the King, just because she thought he might have had feelings for her and orders her to be publicly executed. When facing execution she shows no fear. Ultimately subverted as Blackjack rescues her before the execution is carried out.
- Hair of Gold, Heart of Gold: A very nice person with short blonde hair.
- Nice Girl: So nice that even in the midst of a city-wide manhunt for her, her neighbors refused to turn her in.
- The Scapegoat: Ileosa frames her for Eodred's death, partly out of jealousy of the late king's affection for Trinia.
