In flashbacks, Eko, as a priest, is asked to investigate a potential miracle in Sydney, of a girl named Charlotte Malkin who seemingly died and returned to life. Charlotte's father Richard tells Eko that he is a fraudulent psychic, and that there was no miracle. Charlotte meets Eko at the airport, and passes on a message from Yemi in the afterlife.

On the Island, Eko sees Yemi in a dream. He tells him to make Locke take him to the Question Mark. Jack, Kate, Locke, Sawyer and Eko discover Michael at the Swan, who claims Henry shot him, Ana Lucia and Libby and escaped. Libby turns out to be still barely alive. Eko asks Locke to lead him to the Question Mark under the pretense of tracking Henry. Locke's map leads them to the beechcraft, where they find a new DHARMA Station, the Pearl, which shows surveillance feeds of the other stations. The Pearl's orientation video states that it is designed to observe a "psychological experiment" at the Swan. A disgusted Locke concludes that the button is meaningless, though Eko is convinced to start pushing it in his stead.

Sawyer gets the heroin to ease Libby's passing, allowing Kate to see where he has the guns. The two inform Hurley about what has happened to Libby. Libby dies, after trying and failing to warn the others about Michael.

* ArbitrarySkepticism: Locke is dismissive of Eko saying he's being guided by a dream, even though it was a dream that made him find the beechcraft in the first place. Justified, as Locke is in the middle of losing his faith in the island.
* CallBack: After watching the Pearl orientation video, Eko asks Locke if he wants to watch it again, just as Locke said he needed to see the Swan video again in "[[{{Recap/LostS02E03Orientation}} Orientation]]". This time, a disillusioned Locke says he's seen enough.
* DiesWideOpen: Libby dies with her eyes open, with Jack performing the respectful closing.
* DueToTheDead: Eko says a prayer for Ana Lucia after finding her in the hatch.

* TheReveal: Richard Malkin admits that he's a fraud to Eko, gathering info on those who visit him and then telling them what they want to hear.
* SadTimesMontage: The episode ends with the various characters in the Swan breaking down over Libby and Ana Lucia's deaths, Locke looking totally defeated at discovering the button is meaningless and Michael quietly stewing in self hatred over the murders he has committed.
* WakingUpAtTheMorgue: Eko's flashbacks see him investigating a girl who supposedly drowned and woke up during her autopsy. The audience is treated to the undertaker's recording of what happened which is so horrible that the man ''begs'' Eko to take the tape away so he never has to hear it again.
* WoundedGazelleGambit: Having shot himself in the previous episode, Michael uses this to pretend that Henry shot him in his escape and maintain his cover.
