In flashbacks, we are shown the story of Jack's wedding. At the rehearsal dinner, Jack's fiancée, Sarah, tells the story of how he fixed her when she broke her spine in a car accident. Jack struggles to write his vows, which his father tells him is because he's no good at letting go. At the wedding, Jack tells Sarah that she was the one who fixed him.

On the Island, Jack frantically tries to save Boone. Claire starts to go into labor, and with Jack occupied with Boone, Kate has to deliver the baby with Charlie and Jin's assistance. Sayid and Shannon are off alone on a date, with Shannon blissfully unaware of the danger her brother is in. Boone needs a blood transfusion, which Jack, who is a Universal Donor, uses his own blood for. When the transfusion doesn't work, Jack says they need to amputate the leg, but Boone convinces him to let him go. Claire is terrified that the baby will know she wanted to give it away, but Kate reassures her that all the survivors will help her. As Claire gives birth, Boone passes away. The next morning, Shannon and Sayid return and hear the news, with Shannon crying over her brother's body. Jack tells Kate that Boone was murdered by John Locke.

He initially asks Charlie to ask all of the other survivors to see if anyone's a perfect match for Boone (who's A negative), but it turns out only four of them know their type (and one of those isn't even sure).
