* In ''Anime/{{Monster}}'', Tenma is brought to the sick ward because he had collapsed from not eating. The viewer's immediate reaction is exasperated, because Tenma ''always'' [[ForgetsToEat fails to eat]], but in this case it was intentional, as it allowed him to speak to the man whose help he needed to break out of prison.
* In ''Another Monster'', it is revealed that Nina celebrates her birthday in May. This doesn't seem that special at first, but depending on whether or not this is the real month of her birthday (and by extension, Johan's) and what day in May their birthday falls on, this may or may not be FridgeBrilliance: If the twins were born between May 21 and May 31, then their Zodiac sign would be [[WesternZodiac Gemini, aka The Twins]].
** This [[http://erich-springer.livejournal.com/63759.html page]] argues that the twins were born on the 10th of March, making them Pisces... whose symbol is two identical fish forever chasing each others' tails.
* This one is a bit of FridgeHorror: Mr. Grimmer admits as an jocular aside to a group of orphan boys about his bed-wetting issues as a child. [[spoiler: It isn't until later that we learn he actually was a product of [[OrphanageOfFear Kinderheim 511.]]]]
* After finding out Johan specifically wants Tenma left alive, I was confused as Johan never seemed to do anything to ensure Tenma's survival. Then I remembered Tenma's conversation with [[spoiler:Roberto]]. At no point are we ever given even a rough idea of how many followers Johan has. There could always have been someone there to make sure Tenma lives.
* Johan's eerie imitation of everyone's favourite bedtime storybook, ''The Monster Without a Name''. The monster goes through the village in search of a name, taking on the names of various people willing to "take him in" (literally) before killing them. Johan does exactly the same with his string of foster parents: he manipulates his way into being taken into their family unit, takes on their names, then butchers them from the inside.
* The email received by Nina featuring [[spoiler: Johan's]] attempt at poetry '' 'I was born to smother you with roses' '' may have been an allusion to [[spoiler: The Red Rose Mansion]]
* Grimmer is not the only [[StepfordSmiler constantly smiling man]] in this series - Roberto smiles pretty much all the time as well. [[spoiler: Which makes sense, given that they're both Kinderheim 511 alumni.]]
* Grimmer's name suggests that he's rather "grimmer" than his smiling face would suggest, but it fits in more than one way. ''Grimr'' is a name of the Norse god Odin, and it means "mask". Grimmer's smiling face is a mask for his true emotions!
** His given name, Wolfgang, means "the wolf's path"; fittingly, he experiences werewolf-like shifts to his violent alter ego.
* A mix of FridgeHorror, FridgeBrilliance, and ShownTheirWork: Throughout the series, it is hinted at that Johan is supposedly the story's version of The Antichrist. According to some legends of The Antichrist, he is supposed to suffer two head injuries before going on his ultimate rampage. [[spoiler: Johan is shot in the head twice and ''only'' twice in the series in the series. (Once in the beginning, and once at the end.) The credits of the final episode end with a static shot of the [[OhCrap empty bed where the supposedly comatose Johan was previously]].]]
** Tenma's design and personality are quite Christlike, as is his kindness and compassion towards all characters [[spoiler: including Johan]]; Roberto serves as the lesser-known Second Beast, whose role is to speak for the main Beast and compel others to worship him.
* Inspector Lunge's staunch belief that the murderous Johan is really Tenma's DID alter seems ridiculous... until [[spoiler: more of Johan's dissociated peers keep showing up in the story and Franz Bonaparta reveals that ''many'' of his children developed violent alters, like Grimmer's, and died before the story started]]. Lunge has probably seen a dozen cases just like this.
* The "monsters" from Johan's storybooks look like antisemitic caricatures. Which makes sense, because they were written as part of a [[spoiler: Nazi eugenics program]].

* Just what exactly was it that Franz Bonaparta wanted to achieve? Did he really try to create what Johan became? And if so, WHY because there is no way a creature like Johan can be controlled once he gets out in the world. Could it be it only was [[{{ForScience}} an experiment for the heck of it?]]
* How does Johan have such a perfect face and hairstyle? Brain surgery leaves nasty scars that can't grow hair. Based on the panel showing him after surgery, he should have a mark on his forehead and a bald stripe messing up his bangs.
