In FinalFantasyVI, Celes' suicide attempt at the beginning of the World of Ruin, if you fail to save Cid, all set to the amazingly beautiful "Aria di Mezzo Caraterre". Made just that much worse if the player only found out that it was possible to save Cid after the fact, making it your fault.
In Final Fantasy IX, when you rescue Dagger (Garnet) after her Eidolons are taken away. More exactly, when after the fight with Beatrix, said boss pulls a Heel Face Turn and desperately tries to wake up Dagger from that enchanted sleep, but it looks like it doesn't work. (Luckily, she gets better)
The ending of Final Fantasy X(although it may double as a Crowning Moment of Heartwarming when Tidus meets Auron and Jecht on the Farplane.)
Aeris dies.
At the end of Final Fantasy VI, when Shadow accepts death in Kefka's Tower as payment for his sins.
The ending of Final Fantasy 10. Yuna basically kills all of her Aeons, sends Auron, and then finds out that the love of her life was just a dream all along. Tidus fades away, all the while "To Zanarkand" plays. "The people or the friends that we lost, or the dreams that have faded... Never forget them."
Deleted, wrong series.
In Final Fantasy VII when Aeris is killed. The most strikingly sad moment is when her body disappears into the darkness of the water set to the simultaneously heartbreaking and wistful Aeris theme.
When AERITH dies.
