[[WMG: Neil is dying.]]
If you pay close attention to Neil's sprite at the end when the [[spoiler: red flash]] happens, it looks like he takes painkilers... For what? Also, if you pay close attention to other little moments in the game, like when after [[spoiler: Eva moves River from Johnny's memory]] and Neil calls her a poser for visualizing a door after telling him she still had something to do, she wants to know why he's being so melodramatic and fighting so hard... and Neil doesn't respond... It hints that he's hiding something. Of course he ''could'' just be addicted to painkillers...
* Also, if you select Neil to explore the house at the beginning of the game, when the fake RPG battle happens his HP is not at full like everyone else's, also hinting at illness.
* Former confirmed in ''Impostor Factory:'' [[spoiler: Neil's mother, Lynri Watts, had a neurological condition that could turn deadly at any time. When she was pregnant with Neil, she had a bad neurological attack, causing her condition to worsen. Given the choice to induce a premature birth to receive treatment within the window or carry the baby to term but expiring the treatment window, Lynri chose to carry to term. As honor of this sacrifice, Neil's father Quincy Reynard, suggested for the baby to take his [[NomdeMom mother's surname]]. Lynri would live only for a few more years before succumbing to the condition and Neil would later inherit it. Painkillers are presumably to treat the condition's effects. That moment when Eva asked why he was fighting so much against her actions to change Johnny's memories? He was watching a ''dying'' man lose one of the greatest people in his life (River) possibly forever in memory, something that presumably hit way too close to home for Neil. His geek and LargeHam tendencies? All of it could be all a cover to live life as if it is his last day because for Neil, ''it really could be.'']]

[[WMG: The reason why Johnny is called Joey.]]
As stated by the game, Johnny and Joey were identical twins. Since Joey died in an accident and Johnny's memories were repressed intentionally, it's possible that their mother purposely called Johnny by both names to avoid him asking questions if people called him by the wrong twin's name. The story that the nickname was after his grandfather was likely a cover story in regards to this.
* Confirmed by Eva, although it's just a very educated guess on her part.

[[WMG: Eva is dying, and Neil is trying to reconstruct her memory, but not according to her wishes.]]
In the real world, Neil loved Eva, but she never returned his affections. So, when she decides to have Sigmund Corp. fulfill her wish, Neil takes the opportunity to relive some of their previous cases and alter things slightly to cast himself in a better light than Eva originally perceived. THAT is why Neil seemed to be so invested in Johnny's fate--to make himself seem like a more caring and sensitive person to Eva. Him sneaking off towards the end of the game had something to do with him managing the equipment hooked up to Eva. As for the stinger, the red flash was a sign of Eva's deteriorating condition, and the painkillers? Well, Neil's been working on the memories for several days now, and he needs to take something to be able to keep at it.
* I agree about the Eva is dying part - the part that convinced me was when Neil suddenly had to "feed ist digital pet" and ran off without explanation, presumably to take care of something in the real world or to alter some important detail of Eva's memory he had not thought of before. But I thought he had a different motivation: my guess is that this particular case was not solved, and in reality they never did manage to rework Johnny's memories to get him to the moon. This, maybe in combination with some other unsuccessful cases (which will be handled in the sequel(s)), filled Eva with guilt and selfdoubt, so Neil is in the process of reworking her memories so that she doesn't think of herself as a failure anymore and instead believes she helped all of her patients to die happy. This takes a lot of time and effort, which in turn is taking it's toll on Neil, who takes pills to keep himself awake. The red flash at the end was a sign of Eva's condition getting worse.

[[WMG: The Sigmund Agency of Life Generation is just a scam, and hastens the death of the dying patients.]]
It is heavily believed that anyone who died with any of the employees of the agency doing work on them died with the memories and dreams they want. However, because we can't just ask any person who died to see if their memories were actually changed or not, the entire ordeal of changing a person's memories might be all pointless. As far as the players know, let alone the employees who do the work, we see all the changes in their memories, but it wouldn't be certain if those memories REALLY ARE exchanged for their current memories. Not to mention, the effect of "having your memories being changed" is essentially death according to the description of the game on Freebird Studio's website, so it can't even be tested to see if there were actually memories being made or if the dying patients were just watching a video of how they want their lives to play out before the helmet and equipment being used kills them in some way.

[[WMG: The cases that have been/will be presented to the player are actually the cases where Eva and Neil have failed to create the patient's wish.]]
With the recent minisode in mind, it's foreshadowed that we are either going through Eva's or Neil's (more likely Neil's) set of memories. It might just be pessimism, but it's possible that all the stories, To the Moon, and the next cases that have yet to be released, are as fake as the memories created for Johnny's happy ending. And remembering how Neil truly feels about his job (not in exact words, but something along the lines of "I want these geezers to die freaking happy"), it makes sense that he wants to change all of his failed cases into successes before he passes. And of course, it could be very similar with Eva being the patient instead.

[[WMG: Neil's illness involves short-term memory loss.]]
In the most recent minisode, Neil a) appears to be doing something with the Foundation's equipment, and b) has his name down as shutting down the power... despite, as far as he knows, not having done so. If Neil had short-term memory loss, likely as a result of his illness, that would explain him not being able to remember why he shut down the power, and also provides a possible explanation as for why he's messing with the memory machine - he's trying to figure out if he can access his own memories, or look at his memories from a different perspective to try to solve the problem.

[[WMG: In the second minisode, someone's trying to sabotage Sigmund Corp. to get the agency shut down, and Eva is investigating the saboteur.]]
We know, thanks to the first minisode and ''Finding Paradise'', that there are people who are against the work Sigmund Corp. and other agencies like it do. One of these anti-Sigmund people could have stolen a machine and Neil and Eva's login info, which explains why Eva saw the apparition of herself (the saboteur was using her image to project their consciousness into the real world) and why the computer claims that Neil was the one to shut off the power even though he was in his office at the time. At the end of the second minisode, we see Eva hooked up to a machine because she wants to figure out who's behind the sabotage.

[[WMG: Neil has a chronic pain condition.]]
