The tl;dr is that Heiter is giving Jenny, Lindsay, and Katsuro an explanation ''in graphic detail'' exactly how he's going to attach each "segment" from one's mouth to another's anus, making a complete digestive tract.
18	* Jenny sobbing brokenly for her mom and begging Lindsay for help as Heiter is forcefully sedating everyone before surgery. Shit in the mouth is one thing, but that kind of raw despair? ''That'' is disturbing.
19	* Heiter himself is a NightmareFuelStationAttendant. A deranged MadDoctor who loves to inflict pain to innocent people and then playing with them simply for giggles. The part when he explains the operation to his captives is quite unnerving. You can see pure evil madness in his face.
20	* Even the poster is disturbing. It depicts a man on his knees with a look of pure agony on his face with two extra pairs of hands, pressed up against a pane of frosted glass that's covered in bloody handprints.
