* ThatOneLevel: The Babylon Club front mission is a massive difficulty spike considering that it is an EscortMission. It involves you to protect the VIP and escort it to the door. The enemies appear in [[ZergRush infinite waves]] and you're welding a Desert Eagle which although it is one of the most advanced than the Automatic 45 and deals more damage than the former, it has a very slow rate of fire which thankfully it is possible to also wield the Micro SMG, but i doesn't ease the mission not even in [[SuperMode Blind Rage]] mode as the enemies can not only easily kill you if you don't pay attention to the health bar and activate the "MEDIK" cheat, but your VIP too.

* DemonicSpiders: {{B|FG}}azooka, {{chainsaw|Good}} and GrenadeLauncher users all can OneHitKill Tony. Also the {{Pirates}} in the boat smuggling runs as they can easily destroy the boat Tony has to use.

* GoodBadBugs: The Microsoft Windows version has some humorous audio quirks (especially on slower computers), such as enemies screeching in [[HeliumSpeech extremely high-pitched Spanish]] during combat.

** Rival gangs attacking your businesses when you are collecting money from them. For no clearly explained reason you can only do this as a full round where you pick up money from ''every'' business in one go. It's not so bad when you have taken over only the left half of the map. Once you have spread to the other side, there is a good chance a business will be attacked so far away from your current location it is literally impossible for you to get there on time to save it. You can hire guards to protect your businesses, but they never manage to repel attacks by themselves and only serve to buy you time to get there and handle things in person. Once a business has been wrecked you don't get money from there, and have to spend more to repair it and put up new protection. As a cherry on top, there is the separate activity of taking over territories from gangs. Even if you have taken over 100% of the gang territories, somehow that does nothing to prevent the attacks during money collection.
