Ultra-failed attack!", which does reduced damage, which becomes hilarious if you managed to kill the target of your ire anyways. However, it's also possible to get tainted medicine - instantly killing you. Hacking can cause your head to explode if the opponent counterhacks you. And some of the psychic abilities have the change to spontaneously kill you.
