It's almost never a stroke (which produces such effects in real life), because [[DudeNotFunny that's not exactly comedy fodder]] - and as [[SixFeetUnder the trope namer]] for {{Narm}} demonstrated, this isn't an easy trope to [[PlayedForDrama Play For Drama]].
