* Subverted in ''Manga/{{Ojojojo}}''. Chris is referred to by his first name despite using LastNameBasis for everyone else (even [[OfficialCouple Haru and Tsurezure]] use each other's last names). But it turns out later on that, because he introduced himself in Western name order (since he's British), they all thought that Chris was actually his last name.

* In ''Literature/SoundEuphonium'', there's a fateful moment when Reina and Kumiko hike up a mountain together. Before this, they refer to each other as "Kousaka-san" and "Oumae-san"; afterwards, they drop the honorifics and become much closer, referring to themselves by first name only.

The notable exception is Tarou Takanashi, who is referred to by his first name by nearly everyone due to [[TheLoad how little respect they have for him]].

* In SchoolRumble Hanai and Mikoto are more on FirstNameBasis with each other since they are childhood friends although they use honorifics in public for the sake of propriety. However during the survival game Hanai screams "Miko-chan!" when Mikoto gets shot. Its such an intimate nickname that it embarrasses the both of them to no end later.

* In the first episode of ''Anime/PuellaMagiMadokaMagica'', the NewTransferStudent Homura Akemi calls Madoka by her first name and asks Madoka to do the same. [[spoiler:This is because in the third timeline, the [[FamousLastWords last thing Madoka said]] before Homura [[MercyKill Mercy Killed]] her was how happy she was that Homura had called her by her first name.]]

** Strictly speaking, not all of the Straw Hats even ''have'' family names, or at least ones we've been made privy to. Nami, Usopp, and Brook don't have the chance to go by surnames, even to strangers. Franky and Sanji's legal names both have surnames, but neither of them use them, since Franky faked his death and [[spoiler: Sanji hates his family too much to want to keep his surname]]. Of the Straw Hats that do have family names, only Chopper uses his regularly, and that's probably just because "Tony-Tony" is kind of awkward to say.
