They typically only go after those who have solved the Lament Configuration or have managed to elude them after opening the box and while they are willing to kill others as collateral, they actively spare those who are truly innocent. Also, simply solving the puzzle isn’t enough to get their attention, there has to be a reason or desire behind it. The sole straight forward subversion of this is Pinhead Unbound and his pseudo cenobites from the third movie.
